Distinguished Alumni Nomination Form

Nominate a graduate of Jacksonville University for one of the following awards:

1. Distinguished Alumni of Excellence, given to alumni who exemplify what it means to be an exceptional Dolphin;

2. Scott Amos Recent Alumnus of Distinction, given to exceptional GOLD (Graduates of the Last Decade) alumni;

3. Outstanding Military Alumnus of Distinction, given to alumni who honorably served in the United States military;

4. Tim and Stephanie Cost Giving Back Giving Forward Award, given to alumni who demonstrate service to the community.

Candidate Criteria:
- The nominee must be a graduate of Jacksonville University.
- Nominations will not be accepted if the nominee has already received the award for which they have been nominated.

Application Instructions:
- Complete the following form as accurately as possible.
- Please verify nominee contact information prior to submitting.
- Submit your nominations by August 8.
